CONCIERGE MEMBERSHIP TERMS AND CONDITIONS
Revolt Healthcare Alliance Concierge Services


  1. Concierge Patient Advocacy services are not an insurance product.
  2. The concierge service is considered a voluntary product.
  3. The price of the service is not bundled with your insurance premiums and is collected separately by Revolt Healthcare Alliance.

 

1. Terms of Service

By activating your Membership (“Membership”) and using the Revolt Healthcare Alliance Concierge, you agree to all of the terms and conditions of this Revolt Healthcare Alliance Concierge Usage Agreement (“Usage Agreement”). If you do not agree with any of the terms or conditions contained herein, please do not use Revolt Healthcare Alliance Concierge services (“Services”). Revolt Healthcare Alliance Concierge reserves the right to change, modify, add to, or remove portions of this Usage Agreement or the terms or conditions contained herein at any time. Changes to this Agreement will be periodically made and included in a new Usage Agreement, which will be posted in place of the old Usage Agreement. Your continued use of Revolt Healthcare Alliance Concierge following the posting of any changes will mean that you have accepted the changes.

 

2. Rate Increases

If Revolt Healthcare Alliance ever issues a rate increase, you will be sent a new subscription agreement. We will never increase your membership rate without your electronic consent and acknowledgment.

 

3. Membership Term and Subscription

Memberships are for an annual term. Your monthly subscription will begin on the date you accept these terms and will renew on the same day every month thereafter until the membership is canceled. Memberships are not set to renew at the beginning or end of each month; rather, they begin on the date specified when your activation email is sent. The beginning date is intended to coincide with your insurance policy’s effective date but may vary.

 

4. Payment Frequency and Rate

Annual memberships are billed monthly. Membership rate is non-prorated at $12.95 per month for individuals and $19.95 per month for families.

 

5. Concierge Business Hours and Response Times

Concierge Services hours are 9:00 A.M. to 4:00 P.M. CST Monday through Friday.

Revolt Healthcare Alliance agrees to provide members with services for finding the best pricing on labs, imaging, and prescriptions by the close of the next business day of the members’ request for such pricing.

For doctor appointments, surgeries, and medical procedures, Revolt Healthcare Alliance is dependent on response times from providers and facilities. Revolt Healthcare Alliance agrees to generate proposals and send them to facilities or providers for their response by the close of the next business day following the request for services. We cannot guarantee how long it will take for proposals to be returned.

 

6. Membership Renewal

Memberships renew annually on the effective date of your membership.

 

7. Services

Revolt Healthcare Alliance Concierge services are patient advocacy services for medical service repricing, medical bill collections and negotiations, healthcare system navigation, and other services and activities by which Revolt Healthcare Alliance assists members by acting as the member’s advocate or concierge in helping them access quality healthcare while responsibly addressing financial matters related to their healthcare. Detailed services include:

  • Explanation of benefits and coverages
  • Finding in-network facilities and hospitals
  • Finding the best prices for prescription drugs, labs, and imaging services
  • Coordinating care for complex medical issues
  • Coordinating the transfer of medical records, x-rays, and lab results
  • Pre-negotiate with providers and facilities for upcoming medical procedures and services
  • Post-billing negotiation
  • Claims explanation and support
  • Doctor look-up is not included (this service is provided directly by your agent)

 

8. Savings Disclaimer

Pre- and post-bill negotiations are on a best effort basis, and Revolt Healthcare Alliance Concierge offers no guarantee of exact savings or reductions in fees or bills.

 

9. Cancellations

Memberships are cancelable with 30 days’ notice. To cancel your membership, email concierge@RevoltHealthcare.com.

 

10. Refunds

There will be no refunds. Use of the membership signifies acceptance and understanding of these terms and conditions.

 

11. Privacy

Revolt Healthcare Alliance Concierge knows how important privacy is to you and is committed to honoring your privacy. All personal and financial information obtained is subject to HIPAA laws. Any information collected and disclosed to a third party will be for the sole purpose of processing concierge requests and will be handled according to HIPAA laws.

 

12. Limitation of Liability

In no event will Revolt Healthcare Alliance Concierge, any of its partners, providers, affiliates, agents, including their respective officers, directors, employees or representatives, be liable for any indirect, incidental, compensatory or punitive damages or damages resulting from loss of monies arising out of the use, inability to use, or the results of use of the Revolt Healthcare Alliance Concierge. You agree to indemnify, defend and hold harmless Revolt Healthcare Alliance Concierge, its service providers, and each of their subsidiaries, affiliates, officers, directors, shareholders, beneficiaries, members, partners, employees, consultants, attorneys and agents and their respective successors and assigns, if any, (collectively the “Indemnified Parties”) from and against all claims, actions, losses, liabilities, damages, costs and expenses (including, but not limited to, attorneys’ fees and costs) arising from or relating to your use of the Services. The Revolt Healthcare Alliance concierges’ comments and recommendations presented to members are based on research and opinions collected by the concierges and are subject to change at any time. In addition, all responses will be provided based on reasonable research and efforts on behalf of our staff. No guarantees can be given about the effectiveness of the Revolt Healthcare Alliance Concierge service.

 

13. Notice

All notices to a party shall be in writing and shall be made via U.S. mail at the address set forth below, or the member’s address on file, upon acceptance of this User Agreement.
